    To enroll your child in our preschool program please download, print and fill out the application and bring your completed forms to Desert Star Academy Preschool located at 5635 Highway 95 Suite F, Fort Mohave, AZ.  Hours are 7am to 5:30pm Monday-Thursday.  Any questions please call 928-788-3987.

  • Elementary & Middle

    Download and Print the Enrollment forms to register your scholar for Desert Star Elementary (K-5) and/or Desert Star Middle School (6-8)

    Download and Print the Enrollment forms to register your scholar for Desert Star High School.  We are currently only enrolling for grades 9th and 10th for the 2024-2025 school year.  Grade 11 will be added for the 2025-2026 school year and Grade 12 will be added for the 2026-2027 school year.
